Mainly three things: who controls the wallet the funds landed in (recovery is much more likely when an exchange controls it), whether the case is technically recoverable, and how quickly you report it.
Some cases are genuinely beyond recovery — for example funds sent to a burn address, locked in a smart contract, moved between incompatible chains, or sent to a private wallet, another exchange, or a third party we don't control.
